About the FMEA Simulator

A conveyor with roller drive, product sensor and safety guard is paired with a risk-review workstation. Select a failure row, edit ordinal severity/occurrence/detection ratings, and observe how mitigation changes RPN without erasing severity.

What the simulator shows

• 3D scene parts: Conveyor rollers / belt; motor drive; product sensor; guard / interlock; fmea risk-review workstation. • Controls: Reviewed failure mode (Drive seized, Sensor missed product, Guard interlock stuck); Severity rating (1–10 ordinal); Occurrence rating (1–10 ordinal); Detection difficulty rating (1–10 ordinal); Apply occurrence / detection improvement. • Live readouts: Risk priority number; Effective occurrence rating; Effective detection rating; Severity rating (ordinal). • Guided experiments: Mitigate detection / occurrence; High severity remains. • Four tabs (visual laboratory, curves and measurements, experiments, learn and assess), a model-verification run, a timestamped event log and a trial report.

The model equations

RPN = severity × occurrence × detection Mitigation: Oeff=max(1,O−2), Deff=max(1,D−3) Severity is unchanged by this occurrence/detection control

Model limits and how to explore

Representative systems-engineering teaching example. Equipment geometry and animation time are illustrative; this is not an equipment qualification, safety assessment or certification tool. Ratings are ordinal illustrative values, not probabilities. RPN is only a prioritization aid; identical RPNs can hide different severity. No acceptability threshold or validated industrial safety rating is implied. Try the preset experiments, then compare the live readouts with the equations.

Frequently asked questions

Does an illustrative passing model certify a real system?

No. The stated model omits real qualification evidence and system-specific assumptions.

Which principle does this laboratory emphasize?

RPN does not replace severity review.

What does this simulator not model?
