The unit under study: the reactor feed system supplying an exothermic CSTR, from atmospheric feed storage through a feed pump and preheater to the reactor's own feed nozzle. Rather than sizing something forward, this project inverts the discipline — the system is already fully designed, and the question is what could go wrong with it, and whether the safeguards already in place are actually sufficient and actually independent of each other.
This module walks the whole study the way a real HAZOP team would: node definitions, a guide-word walkthrough (No/Not, More, Less, Reverse) applied to flow, pressure, and temperature at each node, six worked findings with their risk ranking and recommendation, and the action-tracking discipline — including OSHA PSM's five-year revalidation cycle — that turns a study into real, verified plant changes rather than a report that gets filed and forgotten.
